Indications
- Hypertension
- Heart failure
- Post-MI
- Diabetic nephropathy
Dosing
- Adult
- HTN: 10 mg OD, max 80 mg OD; HF: 2.5 mg OD, max 35 mg OD
- Paediatric
- 6–16 years: 0.07 mg/kg OD (max 5 mg)

Contraindications
- History of angioedema with ACEi
- Pregnancy
- Bilateral renal artery stenosis
- Hyperkalaemia > 5.5 mmol/L
Side effects
- Dry cough
- Hypotension (first dose)
- Hyperkalaemia
- Angioedema
- Acute kidney injury
Monitoring
- eGFR, electrolytes at 1–2 weeks then annually
- BP monitoring
Key interactions
- NSAIDs (loss of BP control, nephrotoxicity)
- Potassium-sparing diuretics (hyperkalaemia)
